Halal Logo Application form

To be furnished on Latter Head

We M/s……………………………………………………………. situated at (office address) AND having our factory/plant at (factory/plant address), wish to print Halal Logo on our product(s).

1. That we are the manufacturers of the Following Product(s):
   (Name of All the products )
2. We wish to Print Halal logo on following products (name of desired products) only.
3. We assured that, there will be no changes brought in Halal logo in future prior authorization of Halal Committee-JUM.
4. If there will be any kind of tempering with logo, Halal committee has right to revoke Printing of Halal logo immediate. Misuse of logo constitutes Fraud.
5. In future, Company will renew same if needed or else will inform Halal committee-JUM in advance and will capitulate Halal logo to JUM. The logo remains property of Halal Committee-JUM. And must be returned upon demand.
6. That the above mentioned product/s does not contain any ingredient/s, which is Haram, forbidden under Islamic Shariah.
